Cazenovia College is pleased to offer our students a Basic Sickness and Accident insurance program. The plan is being offered through Gallagher Koster, Inc. The student will be covered from Aug.18 to Aug. 18 for a fee of $1,150. Fees are prorated for January entrants.
Cazenovia College requires all students to be covered by health insurance. Enrollment in the Basic Sickness and Accident coverage may be waived if you have existing insurance coverage that provides equivalent benefits. Students insured under HMO/PPO plans are urged to review their coverage prior to making a decision about the Basic Sickness plan. Benefits under HMO/PPO plans may not adequately cover a student while away from home, especially for non-emergency (not life threatening) treatment.
The student insurance plan is an ideal supplement and may help protect against any medical expenses not fully covered by other insurance. For more extensive coverage, an Enriched Student Plan is also available. Brochures are available through the Health Office after May 15 or at www.gallagherkoster.com.
Agencies offering public assistance include Medicaid, Child Health Plus, and Family Health Plus. To determine eligibility contact: Medicaid at your local county Social Services Office or Child Health Plus at 1.800.698.4543.
To opt out of the student plan coverage, please complete the online waiver form at https://www.gallagherkoster.com by AUGUST 27.
To apply for the Basic Sickness and Accident coverage, complete the enrollment form on the Gallagher Koster website. Checks should be made payable to Gallagher Koster, Inc. and payment sent directly to the insurance company or to Campus Health Services, 10 Seminary St., Cazenovia, NY 13035.
NOTICE:
THE ONLINE WAIVER FORM or PURCHASE OF A STUDENT INSURANCE PLAN IS A MANDATORY REQUIREMENT FOR ALL FULL TIME STUDENTS.
