Physicals
All entering students are required to have a college admission physical. (Sports physicals done by the high school are not acceptable). The form should be on file prior to the first day of classes. If necessary, physicals are available through the Health Service and can be scheduled after August 15. The fee for college admission physicals is $60 for non-residential students. The fee for sports physicals for non-residential student-athletes is $50; student-athletes may find the Sports Physical form on the Health Forms page.
Immunizations
All students born after 1956 are required to submit proof of immunizations including 2 measles, 1 mumps and 1 rubella shot given after the 1st birthday. New York State law prohibits class attendance if documentation is not provided at the start of classes.
Meningitis Vaccine
Check with your Health Care Provider for more information about the Meningitis vaccine. Vaccines are available through the Health Office.
Health Insurance
Students are required to provide proof of health insurance each year by August 7. A basic student insurance plan is available through Gallagher Student Health & Special Risk. Fees for the 2019-20 academic year are as follows:
- Annual $2,215 (08/18/2019 – 08/17/2020)
- Fall $923 (08/18/2019 – 12/31/19)
- Spring $1,292 (01/01/2020 – 08/16/2020)
This plan can be used to supplement your existing plan. The fee is prorated for January entrants. The plan covers one physical per year. Go to www.gallagherstudent.com/cazenovia for student insurance information.
Prepaid Fees
Prepaid fees cover most services and supplies, however, a separate charge may apply for stock prescription medications, vaccinations, lab tests and special order medical supplies. A statement is available for insurance purposes. Students do not need to be enrolled in the College's student insurance plan to take advantage of the health services on campus.
Because commuter students are not charged prepaid fees, a $20 charge is assessed for visits to the Health Center.

Absences
Students are responsible for notifying professors of absences caused by illness or injury.
